collaborators [1]. Many potential lung transplants are lost because of hypoxemia after apnea test. Th e authors evaluated the positive eff ect of a single recruitment maneuver (RM) after the apnea test with encouraging eff ect. In light of the study from Mascia and collaborators [2], we think that the RM will not be necessary if the apnea test is performed with continuous positive airway pressure (CPAP; with positive pressure applied at the level of the positive end expiratory pressure used for ventilation). Using this method for the apnea test, Mascia and collaborators obtained a better PaO2/FiO2 ratio (the ratio of arterial oxygen tension to fractional inspired oxygen) after the CPAP apnea maneu ver compared to the classical apnea maneuver but with out the hemodynamic risk.

Benefit of a single recruitment maneuver after an apnea test for the diagnosis of brain death
INTRODUCTION Many potential lung transplants are lost because of hypoxemia during donor management. We hypothesized that the apnea test, necessary to confirm the diagnosis of brain death in potential lung donors, was involved in the decrease in the ratio of partial pressure of arterial O₂ to fraction of inspired O₂ (PaO₂/FiO₂) and that a single recruitment maneuver performed just after the apne...
متن کاملFactors Affecting Patients’ Adherence to Continuous Positive Airway Pressure Therapy for Obstructive Sleep Apnea Disorder: A Multi-Method Approach
Background: Continuous positive airway pressure (CPAP) is the gold standard therapy for treating obstructive sleep apnea (OSA) disorder. However, patients’ adherence to its regular use is poor. The present study aimed to determine the adherence rate to CPAP therapy by identifying factors affecting its regular use and its associated problems and discomforts among a sample population in Mashhad (...
متن کاملEffect of recruitment maneuver on hypoxemia during apnea test: after or before?
t of a single recruitment maneuver after an apnea test for the diagnosis of brain death’ by Paries and colleagues [1]. Th e ‘ideal’ apnea test (AT) should permit an increase in PaCO2 (partial pressure of carbon dioxide in arterial blood) but prevent a detrimental decrease in PaO2 (partial pressure of oxygen in arterial blood). We agree that a recruitment maneuver (RM) after the AT can improve o...

1.0 A continuous positive airway pressure (CPAP) or Bi-level Positive Airway Pressure (BiPAP) device requires prior authorization through the Quality and Care Management Division and is considered medically necessary when ordered by a pulmonologist, neurologist, or a physician board certified in sleep medicine for patients with a diagnosis of obstructive sleep apnea (OSA) diagnosed using a clin...
متن کاملAbnormal autonomic stress responses in obstructive sleep apnoea are reversed by nasal continuous positive airway pressure.
Obstructive sleep apnoea (OSA) is associated with abnormalities in autonomic stress tests, which are tests of cardiovascular response in the autonomic nervous system (ANS). The level of abnormality has been related to the level of overnight arterial oxygen saturation (Sa,O2). We have studied ANS function pre- and post-treatment with nasal continuous positive airway pressure (nCPAP) in six males...
